The meaning of FDG PET-CT and sentinel lymph node biopsy in patients with cN0 oral squamous cell carcinoma

Recently, FDG PET-CT has been widely applied to examine the primary region of malignant tumor and metastasis to the lymph nodes or distant organs. Here, we investigated the usefulness of FDG PET-CT and sentinel lymph node biopsy in patients with cN0 oral squamous cell carcinoma. The scope of this study was 43 sentinel lymph nodes from 23 patients with oral squamous cell carcinoma in whom lymph node metastasis was not detected by traditional imaging, and who were examined by FDG PET-CT. After the sentinel lymph nodes were identified and resected, we evaluated the presence of metastasis in them histopathologically. As a result, in 43 lymph nodes, we detected metastases in 4 lymph nodes from 4 patients, but only 1 lymph node from 1 patient was detected by FDG PET-CT. These results suggest that FDG PET-CT and traditional imaging may have limited accuracy for diagnosing cervical lymph node metastasis, and that sentinel lymph node biopsy is also necessary.
