A man was diagnosed with sarcoidosis and followed up without medication. Three years later, he developed hypercalcemia (12.3 mg/dl), urolithiasis and renal insufficiency. Hypercalcemia was treated with prednisolone (PSL) 25 mg/day. The serum calcium level decreased immediately after the PSL treatment. PSL was tapered and stopped in twelve months. Two years after the cessation of PSL, the serum calcium level became elevated (11.9 mg/dl) again. Hypercalcemia was improved by the re-administration of 20 mg/day PSL, bisphosphonate was co-administered, and PSL is being tapered.
