Navigation-Assisted Surgery for Temporal Bone Fibrous Dysplasia With Simultaneous Improvement of Facial Asymmetry and External Auditory Canal Stenosis

Fibrous dysplasia (FD) is a benign skeletal disorder that can involve any bone. In the maxillofacial region, the maxilla is most frequently affected and usually treated for aesthetic reasons, whereas temporal bone FD is rare and typically requires surgery for functional restoration or improvement. We describe a 22-year-old woman with temporal bone FD presenting with facial asymmetry and hearing loss due to external auditory canal stenosis. A combined procedure by plastic surgery and otorhinolaryngology teams was performed. By navigation-assisted 2 separate surgeries, the lesion was precisely contoured with reference to the contralateral side, and the ear canal was enlarged. Postoperatively, temporal and zygomatic symmetry was achieved, and hearing ability was restored. Because surgery in the temporal bone involves proximity to the auditory nerves, high accuracy is essential. Navigation system provides real-time positional guidance, and the mirroring function facilitates symmetrical correction. This case highlights that navigation-assisted, multidisciplinary surgery can yield simultaneous aesthetic and functional improvement in temporal bone FD.
