As vitreous substitutes (synthetic vitreous body), air, inert gas, silicone oil has been used in ophthalmic surgery. Their history, usage and problems are discussed. Also, the history of our research has been mentioned for synthetic hydrogel vitreous body, and show progress in coping with problems. Especially, benefits, the current situation, future prospects of polyvinyl alcohol gel as a vitreous substitute are discussed. As a result, biocompatibility of hydrogel vitreous substitute is not bad in the eyes, if we care for the suitable synthetic formula. This indicates a high possibility of the clinical application for synthetic vitreous body with tamponade effect and stable in a long time without the need for removal.
