This paper hypothesized that sclerotherapy of fetal cystic hygroma might reduce the size of cysts and thus hydrops fetalis might be cured. The experience with two cases of intrauterine treatment to fetal cystic hygroma using OK-432 are reported. Case 1: The cyst was divided into four parts with septum and the size of each part was 3 cm in diameter. Intrauterine treatments were performed twice and were effective. At birth, the neonate had only small skin folds in the nuchal area. However, he developed respiratory distress syndrome and died. Case 2: The cyst was divided into four parts with septum and the maximal size of the each cyst was 8 cm in diameter. Severe hydrops fetalis and bradycardia were also associated. Intrauterine treatment was performed immediately. The fetus developed intrauterine fetal death the next day. The karyotype was revealed to be 45XO. Sclerotherapy using OK-432 is thought to be an effective treatment for fetal cystic hygroma, if the candidate is selected carefully. However, for cases with severe hydrops fetalis, they are recommended to undergo intrauterine treatment with some additional treatment. Further investigation is needed in order to establish the indication of intrauterine treatment to fetal cystic hygroma.
